Brampton seniors can get free homecare through new Osler program

Seniors in Brampton can get months of free at-home care after being discharged from hospital through a new home care program.

The William Osler Health System has partnered with Bayshore HealthCare Integrated Care Solutions for the Osler@Home program, which aims to help seniors leaving hospital transition back home.

Osler began accepting patient referrals for the new Osler@Home program earlier this week and approved patients will receive 16 weeks of provided care.

The program is aimed at assisting with patient recovery while helping address hospital capacity challenges, particularly among seniors who no longer need specialty hospital services but still require support to recover successfully at home.

Staff at Brampton Civic Hospital will identify eligible patients and meet with them and their families to create a customized care plan for each patient to support “a seamless hospital discharge and a continuous care experience at home,” Osler said in a release.

“We are proud to partner with Bayshore HealthCare Integrated Solutions to bring this innovative program to our patients, and support recovery at home with the expert care of Bayshore staff, supported by Osler,” said Clint Atendido, VP of clinical services with the health system.

Osler, which oversees operations at Brampton Civic Hosptial and the Peel Memorial Centre, has also launched the program at Etobicoke General Hospital.
